My VERIFICATION form was returned to me, now what?
In some cases, your form may be returned to you. There are several reasons why this could have happened. Do not fret - in some cases you still may be able to resubmit your form! See below:
You missed the deadline - this means you didn't hear the reminders on the morning announcements, didn't see it on Twitter, you didn't check the top of your verification form 560-51, the website(s) or ask Mrs. Stroot. Unfortunately, you cannot resubmit the form. The good news? You gave your time and efforts to an organization that really needed your help and you got experience you can put on your resume. Great job!
You didn't complete parts of the form - you didn't write your reflection, left off the dates or hours, or you missed getting a signature. No problem! Complete the parts you are missing and resubmit the form to Mrs. Stroot in room 109 ASAP!
Your organization wasn't pre-approved - if your organization is a nonprofit and you forgot to complete form 560-50 before you did your service, you still may be able to resubmit by completing the pre-approval process. See Mrs. Stroot immediately in room 109 so see if you qualify to re-submit.

Your form was forged - Of course I know you would NEVER do this, but it has happened once or twice. Don't do it. It makes it uncomfortable for everyone and forgery on SSL forms is treated the same as Academic Dishonesty which means the infraction will be referred to administration and handled according to MCPS policy. It isn't worth it.
IF YOU RESUBMIT YOUR FORM, YOU MUST DO SO IN PERSON TO MRS. STROOT IN ROOM 109 DURING LUNCH, BEFORE OR AFTER SCHOOL. RESUBMIT YOUR VERIFICATION FORM, THE "NOT APPROVED" NOTICE, AND ANY OTHER INFORMATION YOU NEED IN ORDER FOR YOUR FORM TO BE ACCEPTED.
